Why the Reformer Changes Post-Surgical Rehabilitation

The Pilates reformer is uniquely suited to post-surgical rehabilitation because spring resistance allows load to be calibrated below bodyweight. After ACL reconstruction, a standard leg press at the gym starts at bodyweight, which may exceed the graft's load tolerance in weeks 6–12 of the ligamentisation phase. The reformer can load the leg at 20–30% of bodyweight and progress in precise increments as graft healing advances. This level of control is not available in conventional physiotherapy exercises.

After spinal surgery, the same principle applies. Early post-discectomy rehabilitation requires core activation without spinal loading, a specific and challenging combination. The reformer allows supine leg work, hip bridging with spring support, and prone hip extension while the spine remains unloaded. These are not exercises physiotherapy clinics without a reformer can replicate.
